Saylors Dental Laboratory Rx Form

SDL Connections Rx Form (In recognition of our clients who may occasionally choose to offer low-cost restorations, SDL Connections offers restorations made by our off-shore partner, Core Dental Lab located in Shen Zhen, China.  Saylors Dental Laboratory will not accept any cases for fabrication through SDL Connections without a completed SDL Connections Rx Form – no exceptions.  No work accompanied by a Saylors Dental Laboratory Rx will be fabricated through SDL Connections – again, no exceptions.
